This position is a staff level RN position in a Call Center environment and requires an on-line pre-employment skills assessment. This assessment is free of charge and can be taken from any PC with Internet access. Candidates who meet the minimum requirements will be contacted via email with instructions. Program Area: Condition Care (disease management program for chronic health conditions such as Diabetes, Cardiovascular disease, COPD, Asthma) PT evenings (20 hrs): 4 or 5 hour shifts M-F between 4pm-10pm with every third Saturday. Paid Training: Typically full time Monday-Friday 8:30am-5pm for up to 6 weeks. Evening training may be an option. • Current, unrestricted RN license • 2-3 years of clinical experience with adult population required. Med/surg, ER, ICU or Telemetry areas preferred. • Excellent oral & written communication skills + strong organizational &motivational skills • Strong computer skills • Ability to be licensed in multiple states • Professional certifications, knowledge of care management assessment techniques, & provider & community resource knowledge are a plus • Case Management Certification is a plus.
